As we get older the risk of falls increase most take place within the home environment.
The good news there are some simple changes we can make to prevent falls.
We can help you feel safer and remain independent in your own home.
The Falls Prevention Service
We can give falls prevention advice and support
We can arrange a home visit to carry out a Falls Prevention Check.
The Falls team is guided by the NHS and Lincolnshire County Council.
The Project manager can take referrals from Health Professionals and the General Public.
We have four caseworkers who work in the county, they will contact a client to arrange a home visit and give support.
The Caseworker will complete a Falls Risk Assessment to identify any concerns.
We can then refer to other agencies if requested.
This can include the following;
Age UK services
Adult Social Care
Handyman Services

The Bridge Project
The Royal British Legion
